anchors audit

Dossier of one file’s pending items (gates + doctor), for batch fixing.

Gathers, for one file, EVERYTHING pending on it:
• the quality gates that apply to its kind (what 'check' would run)
• the systemic findings from 'doctor' that cite it (collision, orphan, identity…)
Default: the file itself only. With --impact, includes the nodes on the impact path
(the unit's pieces and what the target propagates/validates) — to fix the whole unit.
Designed for PARALLEL sweeping: one agent per file, 'anchors audit <file>',
fixes all the pending items at once (header, spec, identity, coverage…).
